This is the Beast Double Wide / Jumbo or Beast Spread Sweep Play also known as the Jumbo So Long play since it is usually a touchdown especially at younger ages. I named it So Long since the Beast Back is saying goodbye to everyone on Defense as he scores. This is from the Coach Parker’s Power Wing Beast Offense playbook for youth football.
The Jumbo So Long Sweep play is the wide sweep play out of the Beast Double Wide Formation. The Jumbo set or Beast Spread is the Beast’s main passing formation, since it is Trips to one side. It also makes a good sweeping formation if you have a really fast tailback to run the Sweep as the Beast Back. Switch out slower FB type BBs for a fast scat back to run this Beast Spread Jumbo Sweep. This play sets up the multiple Sweep Option pass plays from this Spread Jumbo set. If your Cetner can directional snap, align Beast Back over the playside guard to accept the directional snap to Sweep faster play side.

You can run this play under center if your Center is not able to shotgun snap.
